Home Dialysis vs. In-Center Dialysis

Understanding your options is the first step toward choosing the dialysis setting that works best for your health, schedule, and quality of life.

Home Hemodialysis (HHD)

Home hemodialysis works the same way as the hemodialysis you would receive at a center — a machine pumps blood through a dialyzer that filters waste and excess fluid, then returns the clean blood to your body. The difference is that it happens in your living room, on your schedule.

Most home hemodialysis programs require patients to learn the procedure themselves or train a care partner, which can feel overwhelming. Crown Dialysis takes a different approach.

With our staff-assisted home hemodialysis, a licensed dialysis nurse comes to your home for every treatment. The nurse handles machine setup, needle insertion, monitoring throughout the session, medication administration, and cleanup. You receive the same professional care you would get at a center — without leaving home.

In-Center Hemodialysis

In-center hemodialysis is the most common form of dialysis. You visit a dialysis center three times per week, where trained staff connect you to a hemodialysis machine for approximately 3.5 to 4 hours per session. A care team monitors you throughout the treatment.

The experience varies widely depending on the facility. At many large centers, patients are treated in open bays with multiple patients side by side, rotating physicians, and limited personal attention. At Crown Dialysis, the experience is very different.

Every patient at Crown receives treatment in a private room with a heated massage chair, personal TV with Netflix and HBO, and free Wi-Fi. Our nursing staff is consistent — the same people care for you visit after visit. Dr. Gupta, a board-certified nephrologist, oversees all treatment plans personally.

Home Peritoneal Dialysis (PD)

Peritoneal dialysis is a home-based treatment that uses the lining of your abdomen as a natural filter. A sterile cleansing solution is introduced through a permanently placed catheter, dwells in the abdomen to absorb waste, and is then drained. Unlike hemodialysis, PD does not require needles.

There are two main approaches. Continuous Ambulatory PD (CAPD) involves manual exchanges several times per day. Automated PD (APD) uses a cycler machine that performs exchanges overnight while you sleep, leaving your days free for work, family, and activities.

Research shows PD is associated with survival advantages during the first two years on dialysis, better preservation of residual kidney function, and improved transplant outcomes. Because it avoids needlesticks, patient anxiety is reduced and vascular access sites are preserved for future use.

Which Option Is Right for You?

Home Dialysis May Be Best If You...

  • Want to maintain your work schedule or daily routine
  • Value privacy and the comfort of your own home
  • Prefer flexible treatment times (early morning, evening, overnight)
  • Want to avoid traveling to a clinic three times a week
  • Are motivated to take an active role in your care
  • Have a stable home environment with water and electricity access

In-Center Dialysis May Be Best If You...

  • Feel more comfortable with medical staff nearby at all times
  • Prefer not to have medical equipment in your home
  • Enjoy the social interaction with other patients and staff
  • Have a medical condition that requires closer clinical monitoring
  • Want a clear separation between treatment time and home life
  • Appreciate Crown's luxury amenities and private rooms

Why Not Both?

Life changes, and your dialysis needs may change with it. One of the advantages of choosing Crown Dialysis is that you are not locked into a single modality.

All modalities under one roof.

Crown offers in-center hemodialysis, staff-assisted home hemodialysis, and home peritoneal dialysis — all managed by the same nephrologist, Dr. Bharat K. Gupta.

Seamless transitions.

If you start with in-center and later want to try home dialysis — or vice versa — your care team already knows your history, labs, and treatment goals. There is no need to start over with a new provider.

Flexibility as your life evolves.

Recovering from surgery? In-center may be better temporarily. Got a new job? Home dialysis can adapt to your work schedule. Crown supports you through every stage.

The Crown Dialysis Advantage

Most dialysis providers specialize in one modality or one setting. Crown Dialysis is different. We offer the full spectrum of dialysis options — and we do each one exceptionally well.

Our in-center facility features private rooms with heated massage chairs, Netflix, HBO, and free Wi-Fi. Our home hemodialysis program provides a licensed nurse for every session. Our peritoneal dialysis program includes thorough training and ongoing home support.

No matter which path you choose, you receive care from a consistent team led by Dr. Bharat K. Gupta, a board-certified nephrologist who treats every patient personally — not through rotating physicians or mid-level providers.

Not sure which option is right for you?

Schedule a consultation with our team. We will review your medical history, discuss your goals and lifestyle, and help you choose the modality that gives you the best quality of life. There is no pressure — just honest guidance.

Frequently Asked Questions

Is home dialysis safer than in-center dialysis?
Both settings are safe when managed by a qualified care team. Home dialysis patients are trained thoroughly, and with Crown's staff-assisted home hemodialysis, a licensed nurse is present for every treatment. In-center dialysis provides the security of having medical staff and emergency equipment on-site. Your nephrologist will help determine which setting is most appropriate based on your medical history.
Can I switch from in-center to home dialysis (or vice versa)?
Yes. Your dialysis modality is not a permanent decision. Many patients start with in-center hemodialysis and later transition to home dialysis — or the other way around. At Crown Dialysis, all modalities are managed by the same nephrologist, Dr. Bharat K. Gupta, which makes transitions seamless. Your care team will help you decide when a change makes sense.
Which type of dialysis has better outcomes?
Research shows that peritoneal dialysis is associated with survival advantages during the first two years on dialysis and better preserves residual kidney function. Home hemodialysis can offer more frequent treatments, which may improve fluid and blood pressure management. In-center hemodialysis remains a strong option with decades of clinical evidence. The best outcomes come from the modality that you can follow consistently and that your doctor recommends for your specific situation.
Do I need a care partner for home dialysis?
For peritoneal dialysis, most patients can perform treatments independently after training. Traditional home hemodialysis programs typically require a trained care partner. However, Crown Dialysis offers staff-assisted home hemodialysis where a licensed nurse comes to your home — so you do not need to recruit or train a family member.
Is home dialysis covered by insurance?
Medicare covers peritoneal dialysis, including supplies and training. Staff-assisted home hemodialysis is typically covered by private or commercial insurance, managed care organizations, or self-pay. In-center hemodialysis is covered by Medicare, Medicaid, and most private insurance plans. Our social worker can help you navigate your specific coverage.
